Good news! Sorry though that the right cure was found at the wrong end of the season.

Just to add to this we have had the same problem with rotor's flying off. What I found on my engine was the very thin oil pump gasket folded up a very slight edge when pushed onto the oil pump mounting studs. This tiny fold of paper compressed under the pump and held it on a ridiculously small angle which tilted the pump ever so slightly. This was enough by the time it reached the top of the distributor drive gear to lean on the distributor shaft and make it rotate in small circles at the rotor end. Enough to through the rotor out of balance, lift off and hit the cap.
I could take the gasket in and out to prove what the problem was.
I refitted oil pump with some Loctite master gasket and no paper gasket. Problem solved.
